Vitality Health of Utah is a team of mobile physicians who make at-home visits with a focus on aging communities. Our team of experienced physicians and medical staff are dedicated to providing personalized and compassionate care to our patients. We understand that as you age, it can become more difficult to travel to medical appointments, so we bring our services to you. Our team is committed to improving the health and wellbeing of our patients, and we strive to provide the highest quality of care possible.

Shelley Hale-Byington

Shelley Hale-Byington
Nurse Practitioner |

Shelley is a board certified, family nurse practitioner, and is the founder of Vitality Health of Utah. She is passionate about providing patient centered care with a focus on the ‘whole’ person, not just a disease process or symptom. One of Shelley’s strengths is being able to teach in an understandable manner, empowering her patients with the information they need to make informed decisions. She is focused on great patient outcomes that are in line with her patient’s goals for their health. She is committed to taking the time necessary to listen to her patients at their visits, and to identify what they are most concerned about or need help with.

Shelley has been in this industry for over 30 years with extensive experience in patient-centered care. She has worked in several acute care and intensive care settings, both with children and adults, as well as home health and hospice care. Her breadth of understanding of the physical body and disease processes allows her to comprehensively treat and educate her patients. One of her passions is helping people navigate a complicated healthcare system so they can get the best healthcare they need.

In addition to her work as a nurse practitioner, Shelley loves to spend time with her husband and large family. She is a proud mother of 9, Nana to 13 grandchildren, and dog mom to 2. Her husband and family rally around her to support her professionally and personally. In her spare time she loves to watch football, craft, quilt, bake, and play with grandchildren, all while trying to maintain a healthy balance.

Cassidy Warren

Cassidy Warren is a dedicated and compassionate Nurse Practitioner with a strong foundation in patient care, clinical instruction, and health education. She holds a Master of Science in Nursing from United States University and dual bachelor's degrees in Nursing and Community Health Education, bringing over a decade of experience across clinical, academic, and community health environments.

​

Bilingual in English and Spanish, Cassidy is committed to delivering culturally competent care and building trust with diverse patient populations. Her passion for humanitarian service has taken her throughout Latin America, where she has worked extensively with underserved communities. Cassidy is deeply invested in preventive care, patient empowerment, and continuous learning.

​

Throughout her career, she has taken on both leadership and instructional roles, mentoring students and guiding patients to make informed health decisions. Her ability to simplify complex medical information allows her to connect meaningfully with those she serves.

​

Outside of her professional pursuits, Cassidy enjoys spending time with her family, hiking with her dogs, listening to medical podcasts, and exploring new skills.

Adrienne Terry

Adrienne Terry
MSPAS, PA-C

Adrienne Terry is a board-certified Physician Assistant with over 7 years of experience specializing in Endocrinology and Diabetes care. She offers compassionate primary care through both at-home and virtual visits, with a focus on chronic condition management, including diabetes, weight loss, and hormonal health.

 

As a provider, Adrienne is committed to treating the whole patient—not just the diagnosis. She takes time to understand each individual’s unique circumstances and needs, creating personalized care plans that truly fit. “I don’t do one-size-fits-all medicine,” she says. “I know big changes come from small steps, and I’m always cheering on my patients—celebrating both big and small victories.”

 

You can count on Adrienne to be your strongest advocate and to stay up-to-date on the latest research and best practices to ensure you receive the highest level of care.

Adrienne is also a proud mother of five and is married to her best friend and biggest supporter. She looks forward to meeting you soon!

Shylettera Davis

Shylettera Davis

I’m originally from the Emerald Coast of Florida. I decided to pursue a career in nursing after volunteering in an emergency department as a part of an extracurricular program while attending Woodham High School.

​

 

After finishing a baccalaureate degree at Florida State University’s College of Nursing in the spring of 2007 I was able to fulfill my dream of becoming an emergency room nurse at Tallahassee Memorial Hospital. In addition to working in the ER, I worked in various other roles as an RN including home health, hospice, palliative care, and Medicare quality improvement.

​

 

After ten years of working as an RN I decided to pursue an MSN with a family nurse practitioner focus. I was able to achieve this goal at BYU graduating in 2020. After completing the nurse practitioner certification process, I have enjoyed working in both primary and palliative care and being a nursing education instructor in the skills lab and clinical setting.

​

 

My favorite thing to do is travel! I really enjoy the planning process, and I have been very fortunate to be able to visit some fun places with family, and a great group of friends.

Shannon Compton

Shannon Compton
APRN, MSN

I am a Board-Certified Family Nurse Practitioner with over 15 years of experience in intensive care, complex wound healing, and both acute and chronic care. I earned my Master of Science in Nursing (MSN) at Maryville University and am passionate about combining strong clinical skills with a patient-first approach.


As a provider, I am deeply committed to patient advocacy—empowering individuals with the knowledge they need to make informed health decisions and partnering with them to create personalized care plans. I stay up to date on the latest best practices to ensure the highest quality outcomes and to meet the unique needs of every patient.


Outside of nursing, I enjoy an active lifestyle—whether it’s kayaking, riding four-wheelers, or simply spending time outdoors. Most of all, I love making memories with my three children, two grandbabies, and two lively pets.

Jamie Romney

Jamie is a board-certified Family Nurse Practitioner whose passion has been treating patients in their homes with a philosophy rooted in treating the whole patient—focusing not just on symptoms, but on overall health, wellness, and quality of life. Jamie believes that every patient deserves care that is personalized to their unique needs, and she is committed to building strong, trusting relationships with those she serves.

Jamie has been in the healthcare industry since 2008, where she has worked in a Pediatric clinic, Labor and Delivery, a Cardiopulmonary unit, and Home Health. She was born and raised in Utah and has 2 children. She loves to explore the outdoors with her family, participating in hiking, rock climbing, camping and four-wheeling.

Michelle Pattison
Nurse Practitioner

With nearly 30 years of experience in nursing, I bring a deep well of clinical knowledge, compassion, and dedication to every patient I serve. My nursing journey began in 1996 as a Licensed Practical Nurse after receiving my initial training through the Army National Guard—an experience that instilled in me a strong sense of service, discipline, and commitment to others.

​

My greatest strengths lie in my years of hands-on experience, my empathy, and my genuine love for what I do. I am passionate about providing person-centered care and believe it is essential to consider the whole person—not just a diagnosis. Each individual’s medical history, environment, values, and life circumstances matter, and I strive to create care plans that reflect those realities.

​

I have extensive experience working with the geriatric population and truly enjoy caring for older adults. My background includes skilled nursing facilities, long-term care, outpatient settings, home health, and hospice. In addition, my work as a case manager—both with a health insurance company and across long-term care, skilled nursing, home health, and hospice—has given me a broad understanding of healthcare systems and how to advocate effectively for patients across all levels of care.

At the heart of my practice is a simple belief: patients deserve to be heard, respected, and cared for with compassion. I truly love what I do, and it is an honor to walk alongside patients in their healthcare journey.

Growing up in Southeast Idaho shaped who I am. I was raised to work hard, care deeply, and look for ways to lift others up. Even as a kid, I was drawn to helping people feel their best. That passion led me into healthcare early, and I became a CNA at age 15. It’s been the foundation of everything I’ve done since. Desiring to have the mindset of what Nelson Mandela said, “Education is the most powerful weapon which you can use to change the world.” 

 

I connect with people by staying realistic and tuned in to the everyday parts of their lives. I try to lead with common sense, humility, and a genuine interest in who they are. Being practical and down‑to‑earth helps me build trust and show up for others in a way that feels real.

For 15 years, I’ve helped people feel better in their bodies as a massage therapist, and that work taught me so much about presence, intuition, and trust. Becoming a registered nurse opened another door, letting me care for people in deeper and more complex ways. My time in the float pool has been especially meaningful, giving me the chance to learn from many different units and connect with patients from all walks of life. As a nurse, we have the opportunity to heal the mind, soul, heart, and body of our patients. They may forget your name, but they will never forget how you made them feel.” 

 

Once I became a nurse, I knew my path in healthcare was meant to expand. I went on to earn my Nurse Practitioner degree with the intention of practicing Functional Medicine, a model that allows me to treat the whole person and tailor care to each individual’s physical, emotional, mental, or social and spiritual wellbeing. It is a balance prevention as a way of life.

I believe we were born with a body capable of miracles—a mind that can heal, a heart that can love, and a spirit that can soar. I hope I can serve others in the way they need.  Given this opportunity by  Vitality Health of Utah I am excited  serve you.

Chalyse Rodrigues, MSN, FNP

Chalyse Rodrigues is a Family Nurse Practitioner dedicated to providing compassionate, patient-centered care across the lifespan. With over 12 years of experience in healthcare—including many years working directly with patients before becoming a registered nurse and nurse practitioner—she brings a deep understanding of both clinical care and patient advocacy to her practice.

 

Chalyse developed a passion for healthcare early in her career and quickly discovered that patient education and symptom management are at the heart of what she loves most about medicine. She believes that empowering patients with knowledge allows them to take an active role in their health, leading to better outcomes and stronger provider–patient relationships.

 

In her practice, Chalyse strives to create a supportive environment where patients feel heard, understood, and confident in their care plan. She values building long-term relationships with individuals and families and is committed to helping patients improve their health through education, prevention, and evidence-based care.

 

Outside of work, Chalyse enjoys spending time with her husband and their three boys. Their family stays very active and sports-oriented, and she often jokes that her favorite hobbies are simply whatever sport her children are currently playing. Whether it’s cheering from the sidelines or staying busy with family activities, she embraces the joy of an active, on-the-go lifestyle. 

Josh Streeter, DNP, AGNP

Josh Streeter, DNP, AGNP, is an Adult-Gerontology Nurse Practitioner with over seven years of experience delivering comprehensive, patient-centered care across primary care, home health, geriatric, and palliative settings. He is known for his thoughtful clinical judgment, strong communication skills, and ability to build lasting, trust-based relationships with patients and families. Josh is dedicated to providing evidence-based care that reflects both medical best practices and each patient’s individual goals, values, and overall quality of life.

 

Originally from Upstate New York, Josh has lived in Utah since 2018 and takes pride in serving the communities he now calls home. He brings a balanced, practical approach to healthcare, emphasizing collaboration, continuity, and proactive management of chronic and complex conditions. His professional focus centers on helping patients maintain independence, dignity, and meaningful engagement in their daily lives.

 

Outside of his clinical work, Josh enjoys spending time with his family and making the most of Utah’s outdoors through fly fishing, golfing, hiking, and camping.
